Occurrence Summary Date: 2014-10-16 Further Action Required: Yes O.P.I.: General Aviation Narrative: Information provided by Trenton joint rescue coordination centre (JRCC) as to an accident reported at the location of occurrence. A privately registered Heliot, an amateur helicopter, from Lac Manawann, QC to Rivière Falconio, QC sustained an accident, no injuries, extent of damage unknown. Transportation Safety Board (TSB) advised.
Date: 2014-10-17 Further Action Required: Yes O.P.I.: General Aviation Narrative: UPDATE: JRCC Trenton Report#[T2014-02448]:(511712N 0703778W - Riviere Falconio). A 406 emergency locator transmitter (ELT) detection was heard approximately 200 NM north of Bagotville, QC. Subsequent Satellite Personal Tracker (SPOT) beacon activation in the same area. 424 Squadron Hercules R344 and 439 Squadron Griffon R439 were tasked. R344 arrived on scene and located a cooperative target. Search and rescue (SAR) technicians dispatched and confirmed 2 persons on board (POB) and in good condition with no injuries. R439 arrived on scene and evacuated the 2 POBs.

2014-10-21 Further Action Required: Yes O.P.I.: Maintenance (Quebec Region) Narrative: UPDATE: TSB Report#A14Q0161: The Heliot, with one pilot and one passenger on board, was taking off from a wooded area 180 NM NNE of Bagotville, QC (CYBG) for a leisure flight to a nearby lake (unidentified). The helicopter rolled over during initial lift-off and the two uninjured occupants evacuated immediately. The 406 Mhz emergency locator transmitter (ELT)was activated and the pilot also activated a "Spot" beacon, The Joint Rescue Coordination Centre Trenton launched a search party and the occupants were located and airlifted out. The helicopter sustained damage mainly to its rotor blades.
