Occurrence Summary Date: 2014-10-20 Further Action Required: Yes O.P.I.: Commercial & Business Aviation Narrative: A Helijet International Sikorsky S76 (JBA784) from Victoria Harbour, BC (CBF7) en-route to Vancouver Harbour, BC (CYHC), called Vancouver Tower requesting to land Runway 13 at Vancouver, BC (CYVR). Vancouver Tower initiated a precautionary standby. The aircraft landed without incident at 2000Z. No operational impact.
Date: 2014-10-24 Further Action Required: Yes O.P.I.: Commercial & Business Aviation Narrative: UPDATE: TSB Report#A14P0175: The Helijet S76 (C-GHJL), flight JBA784, from Victoria Harbour (CBF7) to Vancouver Harbour (CBC7) was at 3000 feet over Point Grey when the number 2 engine (Rolls Royce 250 C30) lost power. An emergency was declared with Vancouver Harbour Tower and the flight diverted to Vancouver International (CYVR). The aircraft landed on runway 13. There were no injuries. It was determined a fuel governor had failed. The governor was replaced and the aircraft returned to service. A Service Difficulty Report has been filed with Transport Canada.

It is 4th one within 4 years..

One failed in GHJV-medevack machine, second with fire in GHJT, third GHJL, and now GHJL again...

 

How many engine chip lights...no need to know.

UPDATE: TSB Report#A14P0183: The Helijet Sikorsky S-76A (C-GHJL) was on a test flight about 7 nm S of CYVR when the number 2 engine (Allison 250-C30S) experienced an uncommanded acceleration. The crew reduced power to the number 2 engine, declared an emergency and returned to CYVR for an uneventful landing. The number 2 engine and N2 governor had been recently replaced because of an 18 October 2014 power-loss event (occurrence number A14P0175) and engine chip light events on 23 and 25 Oct 2014 (occurrence number A14P0179). Following the 29 Oct 2014 uncommanded engine acceleration event, company maintenance replaced the droop compensator amplifier (p/n 76900-01880-106) and inspected the associated wiring.
