Helicopter crash in Ontario... 4 dead

Somebody GOOFED and four individual's paid for it with their lives.

Why TSB is not releasing the cause is anybody's guess?  

May they all rest in peace and condolences to all Family and Friends.

The names were in the CBC’s article two days ago. They did not publish a cause. You seem ready to assign blame saying somebody “GOOFED”. Perhaps you should let TSB in on what you know

Out of respect to the deceased and their families, let's keep this civil. The families are devastated and any mud slinging will not bring these 4 people back.

Yes someone probably made a mistake. We all make mistakes, we are human. Maybe consider that the mistake was made by one of the deceased. Having that information public while the family and friends are still mourning would serve no practical purpose.

Very, very sad.... hard to see the unfortunate reality at times....

Just has been reported that an unsecured tool bag took out the tail rotor. Also, two workers had not done the seat belt thing.  Check list, check list, check list, check list!
