One take on it is that there is no blame or responsiblility allocated to the individuals involved in an incident. Only insufficient training. Management is responsible for any ill occurance, the CP, CO, whoever is at the top accepts responsibility in theory for everthing from bad luck to bad decision making on all company parties. The first approved/certified SMS in Canada was at 'Guardian Helicopters'. See where this takes us. Insurance Providers have the ball. lineworker Posted October 22, 2006 Author Report Share Posted October 22, 2006 The take described above was given by an SMS trainer making assurances that in SMS there is an investigation as to the factors involved/chain of events leading to an incident. It is not directed at laying blame, though on the Transport Canada website, identifying responsibility for failing to meet goals in the SMS policy is a priority.
