Bell B-206L-1 Jet Ranger helicopter was engaged in VFR aerial spraying in a wooded/agricultural area approximately 45 miles southeast of Cochrane. The aircraft crashed while landing on a truck trailer for refuelling. The helicopter was substantially damaged but the pilot, the sole occupant, was not physically injured. User Name: Donaldson, John Date: 2007/08/10 Further Action Required: No O.P.I.: System Safety Narrative: UPDATE Follow-up information received from Maintenance & Manufacturing [2007/08/10]: A Civil Aviation Safety Inspector (CASI) spoke with the operator's Director of Maintenance (DoM) about this accident. The CASI was advised that the helicopter is a write-off. The accident occurred at GPS co-ordinates: N48.30.16 W079.41.30. More information to follow. Initial Notification (#A07O0217): The Gateway Helicopters Ltd. Bell 206L-1 helicopter was engaged in forestry spraying operations approximately 70NM east of Timmins. After applying the first load of chemical, the helicopter landed on a pad on the refuelling/replenishment truck to get a new load of chemicals through a hose arrangement mounted on the toe of the skid of the helicopter as per an approved LSTC. This arrangement has the helicopter facing the back of the rig. During replenishment, the truck began to roll forward. The pilot lifted off but movement of the helicopter was restrained by the hose (it has a quick-disconnect but does not have a break-away), resulting in the helicopter rolling onto its side near the truck. The pilot was wearing a four-point harness and helmet. He escaped with minor injury. The helicopter sustained substantial damage. There was a small chemical spill that was contained by the operator, but there was no leakage from the helicopter itself. There were no injuries to personnel on the ground. The operator reported they have not previously experienced difficulty of this nature. The truck is equipped with air brakes. As an immediate safety measure, the operator is implementing a procedure to chock the truck before carrying out replenishment operations. XoNe Posted September 4, 2007 Author Report Share Posted September 4, 2007 It looks like someone forgot to put on the air brakes on the truck before getting out to fill up the machine with goop.
