The fin on top is part of the early Bell IFR system as opposed to later models which have the better Sperry system installed.

It's the Dihedral Sail, originally installed on all IFR Bell 212s with the Bell AFCS to improve stability in the roll axis. Sometime in the 90s (I believe) it was allowed to be removed.

 

And carholme - not all late-model IFR 212s had the Sperry. It was optional. The Bell AFCS was produced right to the end.
