Building a human factors course for non Pilots/AMEs

Hi everyone, 

I’m putting together a Human Factors type course for people not fixing and flying aircraft. The students are a mix of tech support and software developers. We want to demonstrate the importance of the information managed in a Maintenance Tracking System. 
 

I’m trying to find a TSB incident report or two to use as a case study of an incident or accident that a major contributing factor was maintenance / inventory documentation or incorrect documentation. 
 

i know I’ve read some in the past, I can’t seem to find them now that I’m looking for them. 
 

Any recommendations?
